One thing I forgot to mention: swapping the bracelet for a strap may also help. The Bulgari Octo straps are quite rigid and this means that the watch case can "float" above the wrist, lessening the pressure of the lugs against the skin.
Both the lugs and the crown cause discomfort for me. Doesn't matter how the bracelet is adjusted, it just changes which part of the wrist is effected. If the bracelet had a half-link or other facility for micro adjustment, I think the problem could be somewhat mitigated.
