From a pure design perspective, I would pick
5178G because the clean dial enables the focus on the MR.
However, I would consider your overall collection to see what fits better in terms of complications and dial color/design.

I would select the one with the crème dial. The blue dial has the perpetual calendar and all but somehow the dial seems to crowded for me

Hit send by mistake. I agree, the simplicity of the 5178 is the way to go. It will never outstay its welcome. Way way above my pay grade, but one can dream! Congratulations whichever you choose!

Dear friends, Thanks indeed for your kind feedback and clear messages. My preference was/is the 5374G, but I will need to think about this based on your unanimous and sound guidance .....
While my preference also rests with the 5178 -- the simplicity and cleanness of this reference are just unbeatable in my view -- you are arguably considering two watches at the very top of Patek's catalog...so there's truly NO wrong outcome here! :-)
Please follow your heart -- and if your initial "clear favorite" was the 5374, then chances are THAT is the right Patek for you! Please keep us posted, and thanks for consulting the community on this most important decision ;-) Cheers.
Allow me to be the contrarian. Assuming in the metal the blue dial is a beautiful as on the 5370P, my strong preference would be the 5374G. Why?
Patek for me is all about complications and to have a MR combined with a perpetual is both practical (the repeater), romantic (the MR) and technically extraordinairy. Blue enamel is just amazing! I also prefer the case style of 5374. For me, again if the ...

Hands down, the 5178G. To those who don’t know what it is, it look like a simple time-only watch. But if you know, you know. So understated and subtle.
