Customs Information for Importing MAD 2 watch to US?

I am in the process of receiving a MAD2 watch from the good people at MB&F, and have hit a snag with US Customs. In the past, Customs (through DHL) has requested my personal information, which I am able to provide. But now they are also asking for "clearance instructions," a "clocks and watches worksheet," and a "value breakdown." That's information that I don't have.

I have put in an e-mail to MB&F, but as I await a response, I figured that someone here may already have gone through the same exercise. If anyone can provide guidance as to what information to use, I'd much appreciate it!

Florida definitely also charges sales tax. They get a report from whatever was declared at US Customs
