RCOPPEL has been really lucky on this one.
I share your concern about this, although I am sure this is an isolated incident ...
I have heard from several owners that first generation deployants had an issue where the pin was not fastened securely enough to the strap and could accidentally open up, but those deployants were switched by RM for better designed ones.
Here, I do not think there is a flaw in the design anymore. More likely, this one passed through the quality control process. Granted: it is scary. Granted: this should not happen anymore and should trigger an improvement in the QA process. But in any case, I am sure people at RM will get to the bottom of this and get it fixed as they always do (I am learning this the hard way, having to let go my watch again for weeks after I experienced a very weird mechanical problem, so that they can perform extensive testing on the movement)
