I had this problem with another watch of mine. I have a special screwdriver I keep
at John Davis's house. He puts one screw driver bit in a vice somehow and holds the watch somehow and tightens the screw somehow adds some lock tight, and it is perfect. 
. He told me he practiced this technique on the 5187 before a tool was designed for this task. And no he doesn't work for Swatch or Breguet, lol. But, if we are nice to him, he might shed some light on ways to change screws when they screw into eachother and need holding from the other side.
