I am not sure about a Ref.3347,the only perpetuals calenders I know of is the 3787 and the 7717,could not find it on Breguet`s web site either.
The seconds hand.I own a 3787 perpetual and am a physician too.I quickly got used to estimate puls frequency without a seconds hand,also probably because many of my dayly worn other watches don`t have one,and if You really want to know exactly,just take 1/2min-You can see the progression of the minutes-hand half way to the next index(takes a bit longer than counting 10 or 15 sec but it works),yet I hardly ever had to do this.Its 36.5mm and 8mm slim,fits perfectly on smaller wirsts.
The bigger tourbillons You mentioned would be a tough choice for me if had the money to buy them:My wrists are not too big too and for this I don`t really like to wear my 42mm/12mm thick watches,but I am sure I could not resist a 3657.The 3757 is even more complicated,but the layout is not so appealing to me.
If Tourbillon watches will suffer less depriciation no one can say,I would not consider this thought in my decision.
The open-worked tourbillon is no classic Breguet design(i.e.19th cent),still extremely pretty to look at.
If I had the choice and could afford,I think I would go for the 3657 for aesthetic reasons although it lacks the perpetual calender.
In any case,try them on first,a visit in one of the Breguet Boutiques(You could contact some of them first if they have the watch in store) is surely worth the journey as,if You are going to spend so much money,You will not like to own a watch that`s too big for Your wrist after all.
