First of all the watches can be seen on the website of Patek. If you click on references in the upper-right corner a list with most references will pop up (you might need to scroll to the right.)
About the watches:
5136: A 3940 on bracelet, hence a perpetual calendar with moonphase.
5036: Annual calendar, moonphase and power reserve. On bracelet.
5037: Annual calendar with 24-hour indicator. On bracelet, diamond set bezel.
