I am tempted to get a 2018 Santos too.. my wrist not big, about 6 inch or so. Rolex SD43 is probably the biggest one I have in my current lineup. Also another thing, not much discount to be offered for a brand new Santos? and not many on pre owned market....
The large Santos is 41. The medium one is 35.
Though being square and on a bracelet makes it look really big. Try both in and see what speaks to you more.
