My choice would be the ALS
I was actually looking at, trying and considering the 5146 a short while ago in both white gold and a more classic colored gold.
A very beautiful case with flowing lines. What turned me off was the dial layout where the three sub-dials being both too close to each other and too close to the centre of the dial. Doesn’t it also indicate a movement that is somewhat small compared to the case;dial?
In the end I went with a Lange - but not the Annual Calendar shown above. I find that to be a beautiful, clean dial that would be a joy to wear on your wrist every day.

This particular annual calendar is very well-balanced. The only minor gripe I have is the interrupted minute track
Patek
I like patek better. The ALS date isn't for me (I know that is the als style but single digit dates look like the watch is winking) and the patek has central seconds. Downfall of the patek is the power reserve.
Tough choice indeed.
I like the ALS for the clean symmetric dial and the view from the back. The 5146 beats the ALS for case + lugs in my view. It wears very well… So it’s a tie!

I think I like the Lange more, but I want to wear the Patek more.
The 5146 case is very wearable and comfortable, and it more easily dresses down with more casual attire.
I'm a Lange fan, but my vote is for the Patek.
But if you compare it with the 1815 AC, the latter will get my vote.

I would choose the Patek over ALS, but it's a close one
Basically, I find the dial of the ALS also very nice, it's just that I dislike the case of ALS, and while I can understand that they stick to it it makes it hard to 100% like their watches.

There may be a time when I have to decide on selling or trading one in. Time will let me know which I favor.
